Richmond West bound, right lane. The is sunk about 2+ inches below pavement creating a pothole like hazard as evidenced by the rubber tire marks on the edge. Manhole cover elevation must be raised to be level with the pavement...just like the other cover next to it in the left lane. Street rebuild contractor tried to slope pavement down to correct for the difference in elevation but was done wrong and created a very bad pothole.
